	Welcome to jay.net a/s In order to get you up and running you need to answer the questions below (1 through 9). After the form is completed Jay.net Network Operations Center will validate it and send you en email where further instructions is available.

	In order for jay.net to assign IP addresses the following questionaire must be completed. All information is treated with the utmost confidentiality. The application is reviewed in accordance to the rules defined by Ripe - further information here: http://www.ripe.net/ripe/docs/iprequestform.html. Ripe acts as the european authority of IP addresses - everybody in europe must be able to document the need for the IP addresses assign to their customers. Generally a 1 IP address to 1 computer ratio is accepted but in cases where technical limitation prevents this a dispensation can be issued by documenting the need and the technical problem. In situations where the customers need is changing the IP Application must be updated and sent to noc@jay.net and NOC will hereafter review and determine if the needs has changed. All questions must be answered in english and if you are in doubt of what to write please state a "?". DIif you have any questions please email noc@jay.net.

5	In case of webservers please state wether theese are name- or ip-based. Namebased means that the server can handle multiple domains refereing to the same ip address. IPbased is where each domain requires an ip address - a problem which occurs on old webserver software versions. In case of ipbased please state why and when you can be expected to upgrade your software which is capable of running as a namebased service.

6	In case of dial in or vpn servers, please state wether theese assigns ip addresses statically or dynamically. Static means that an ip address will be reserved to a specific user - hence the user gets the same ip address at each logon. By dynamically it is understood that the ip addresses are assign in the same way from a pool and it is a random ip address the specific user gets. In case of statically assignment it is required to specify a reason for this.
